EMPLOYMENT FORMS FOR NEW EMPLOYEES. 

This forms needs to be completed by all new employees.

 

State of Michigan W-4.  This form needs to be completed by all new employees.

 

Federal W4 2016. This forms needs to be completed by all new employees.

 

Immigration I9 Form.  This form needs to be completed by all new empoyees.  Complete top section of page 4.  Provide a copy of your driver's license and social security card.

 

New Hire Retirement Plan Election.  This form needs to be completed by new public school employees who first worked on or after September 4, 2012.   If you worked for any other school district  participating in the Michigan Public School Employees Retirement System prior to September 4, 2012, your retirement plan is already in place and you need not complete this form.
